​​Intake paperwork is completed by client or caregiver. Upon submitting an intake form, your family will receive a response from PMT within 2 business days. Treatment is offered at our main location or area facilities & agencies.

If music therapy is recommended, we schedule individual and/or group appointments weekly or biweekly.
